Conferred Master’s degree (transcript)
Resume/CV
Passing score from the NCE
Jurisprudence Exam Certificate
Application for LPC Licensure
Documentation of practicum hours
Supervisory Agreement Form submitted to BHEC
LPC-Associate license received from the state
Professional Liability Insurance (face sheet only; copies only, please)
